Co-Occurring Disorders Treatment

SMG provides a continuum of services for the treatment of substance use and co-occurring disorders

Whether you are struggling with depression, anxiety, or substance abuse, we have trained staff who are here to help you understand your personalized treatment options and embark on the path to recovery.

After an initial assessment, you will have a clear understanding of your situation, whether our setting is right for you, which medications, if any, you should be prescribed, and what your overall treatment plan may look like. Yours may include some or all of the following facets.

Individualized Medication Management

Our skilled mental health providers have years of experience treating a wide variety of issues. Using their knowledge and resources, our psychiatrists and nurse practitioners will determine the diagnosis and psychopharmacological treatment best suited to your needs. We work with you to find and manage a prescription regimen that’s right for you.

Buprenorphine

Buprenorphine is a medication that comes in film, tablet or injectable form. By taking the place of the opioids used by patients, buprenorphine prevents cravings and makes withdrawal from opioids feel less intense. Patients on buprenorphine report feeling normal, functional and clear-headed all day long, which allows them to make positive choices the way that they did prior to their addiction. When combined with counseling and other forms of support, buprenorphine can be highly effective in helping patients recover from addiction. Vivitrol

Naltrexone is a monthly injectable medication that is useful for the treatment of opioid addiction and alcohol addiction. This medication binds to receptors in the brain and prevents patients from feeling any pleasant effects from opioids or alcohol. Vivitrol has been clinically proven to reduce the probability of relapse. Those suffering from an opioid addiction may only start taking Vivitrol after opioid detoxification. Vivitrol may also be used by patients who have completely tapered off of buprenorphine or methadone to prevent relapse.

Individual Therapy

The least intensive level of care is a weekly or bi-weekly meeting with an individual therapist and/or with a psychiatrist or nurse practitioner for medication. Our therapists are trained in Cognitive Behavioral Therapy (CBT) and Trauma Informed Care as well as other evidence based practices to optimize treatment of substance use and co-occurring disorder. Our prescribers are trained and experienced in the treatment of substance use and co-occurring disorders.

Group Therapy

For people looking to maintain their recovery, or wish to focus their treatment on a specific topic, SMG offers a number of outpatient groups. These groups meet once a week for an hour and can also be combined with individual therapy and medication assisted therapy as described above. Examples of groups include: Relapse Prevention, Grief Management, Healthy Relationships, Family Support.

Day Treatment

Our most intensive level of service is the Day Treatment Program (DTP). This program offers a series of groups, Monday through Friday, five hours each day. When combined with individual therapy from a member of our clinical staff and medication assisted therapy from one of our psychiatrists or nurse practitioners, it provides the most comprehensive treatment program available outside a hospital setting. Groups include: relapse prevention, anxiety and depression symptom management, trauma recovery, anger management, DBT basics, healthy relationships and yoga and meditation.

This option is often the next step for someone discharging from an inpatient stay, or for someone who wants to strengthen their recovery.

Residential Treatment

Square Medical Group offers residential services at our location in Waltham, MA. The “River House” program is an intensive 90-day program for individuals with co-occurring mental health and substance use disorders. The program is located in a restored Victorian home situated on the Charles River within walking distance of downtown Waltham.

This is a 16-bed, 90-day residential co-occurring enhanced (COE) program designed to offer support for male adults looking to pursue independent and recovery-oriented living. River House provides a thoughtful treatment environment, offering a robust spectrum of psychiatric and clinical services. These services create a therapeutic milieu resulting in residents gaining greater amounts of self-responsibility and self-efficacy. By addressing our resident’s social determinants of health along with their medical, psychiatric, and addiction recovery needs, they will learn important skills to move beyond their illness, so they can pursue a personally meaningful life.

Requirements for Admission

  • Individual needs to be 18+

  • Co-occurring disorder present, substance use disorder primary

  • Needs to identify as male

  • Symptoms need to be managed well enough to participate in milieu-based residential treatment

  • Must be willing to voluntarily engage in treatment

  • Clients participating in MAT are welcome and eligible as residents

  • Most MassHealth plans accepted
